Vaccine Paves Way for Drug-Resistant Infection

In seven years on the market, the vaccine Prevnar has prevented thousands of serious infections in infants and toddlers and made Wyeth a mint. But the vaccine’s very success in stamping out some common bugs that can cause everything from life-threatening brain infections to run-of-the-mill earaches has opened the door for some rare strains of bacteria to waltz in. And some of them are pretty tough to tame.
Michael Pichichero of the University of Rochester described nine cases of children with earaches that couldn’t be cured with pediatric antibiotics, and needed surgery or a new antibiotic that has not been proved safe in kids. They were infected with bacteria related to those that the vaccine guards against, the AP reported, but the vaccine was powerless against them.
Pinchichero, who presented the results at a meeting of infectious disease experts, wouldn’t discuss the details of the cases with the AP because he has submitted his report to a medical journal. His research was paid for by Abbott Laboratories, which makes antibiotics, and the Thrasher Foundation, which funds projects related to child health, the AP said.
Prevnar helps the body block seven common strains of Streptococcus pneumoniae bacteria (pictured) that cause meningitis, pneumonia and infections of the bloodstream. But by jacking up the immune system to knock out those strains, the vaccine appears to clear the way for other bugs to take over. Several studies suggest that the troublesome strain Pichichero reported, known as 19A, is becoming more common. “It is very worrying,” Keith Klugman, an infectious diseases specialist at Emory University, told the AP.
Wyeth, which sold more than $1.5 billion worth of Prevnar last year, is testing a next-generation version of the vaccine that could protect children against some strains not covered by the current version. The company expects to submit the improved version for approval in 2009. Some observers have suggested that Prevnar could become something like the flu vaccine, which is re-jiggered every year to keep up with the mutating flu virus.
