B-Type Natriuretic Peptide Predicts Severity of Sepsis

NEW YORK (Reuters Health) Mar 24 - B-type natriuretic peptide (BNP) appears to be a useful biomarker for diagnosis and risk stratification of patients with septic shock in the absence of congestive heart failure, US researchers report.

They note in the March issue of the Archives of Surgery that progression of sepsis leads to damage of cardiomyocytes and cardiac contractile dysfunction. Cardiovascular collapse is the main cause of death for the roughly 200,000 fatal cases of septic shock that occur each year in the US.

There is a need for an easy and reliable method of assessing cardiovascular dysfunction in patients with septic shock, Dr. Martin H. Bluth, from SUNY Downstate Medical Center in Brooklyn, New York, and colleagues contend.

"Although other studies have demonstrated BNP as a marker for sepsis and cardiac disease, this is the first study to investigate its use as a potential marker for pure sepsis. Patients with any evidence of cardiac disease were not included in this study," Dr. Bluth told Reuters Health.

The researchers' study involved a comparison of BNP levels in 13 patients with septic shock, 18 with early sepsis, and 18 healthy age-matched controls. All of the septic shock patients were free from heart or renal failure. The patients were followed for up to 21 days and sepsis severity was assessed using the Sequential Organ Failure Assessment (SOFA) scoring system.

Patients with septic shock, but not with early sepsis, had elevated BNP levels relative to controls (p < 0.05). Moreover, the BNP level was directly related to the SOFA score.

Among septic shock patients, a drop in BNP levels during follow-up was predictive of survival, whereas continuation of high levels correlated with death.

However, not everyone is convinced of the predictive value of BNP levels in patients with septic shock. Editorialist Dr. Eddie L. Hoover, from Buffalo VA Medical Center in New York, comments that the researchers provide no data to support their contention that the "BNP level may serve as 'a valuable measure for determining reduced left ventricular function in patients with septic shock'."

He adds that Dr. Bluth's team "can say only that if BNP levels remain high, death is imminent; there is little one can do with such information except withhold treatment, which is not an option in this context."

Dr. Bluth emphasized that "these studies are preliminary and further investigation is necessary before the utility and application of BNP in sepsis is clarified."
