【medical-news】过去三十年多发性骨髓瘤的存活率
NEW YORK (Reuters Health) Jun 21 - Survival of patients with multiple myeloma has improved significantly during the last three decades, investigators in Sweden report.
Dr. Magnus Bjorkholm, of Karolinska University Hospital in Stockholm, and colleagues have published their findings in the May issue of the Journal of Clinical Oncology.
The researchers conducted a large population-based cohort study to define patterns of survival among patients with multiple myeloma diagnosed during a 30-year period in Sweden.
A total of 14,381 patients (7643 males and 6738 females) diagnosed between 1973 and 2003 were included in the study. The mean patient age was 69.9 years old. The team used relative survival ratio (RSR) as a measure of patient survival.
There was a significant improvement in 1-year survival in all age groups. The 1-year RSRs increased from 0.73 between 1973 and1979 to 0.82 between 1994 and 2003.
Improvement in 5-year RSR was restricted to patients younger than 70 years at diagnosis, according to the authors.
Improvement in 10-year RSR was confined to patients younger than 60 years at diagnosis. "This was reflected further in a 29% lower 10-year excess mortality among multiple myeloma patients diagnosed from 1994 to 2003 compared with those diagnosed from 1987 to 1993," Dr. Bjorkholm's team reports.
Females had an overall 3% lower mortality than males.
The investigators note that factors that have probably contributed to improved survival are high-dose melphalan with subsequent autologous stem cell transplantation, thalidomide, and a continuous improvement in supportive care. However, they add, new effective and tolerable agents are needed to improve survival in older patients.

Improvement in 5-year RSR was restricted to patients younger than 70 years at diagnosis, according to the authors.Improvement in 10-year RSR was confined to patients younger than 60 years at diagnosis.

The investigators note that factors that have probably contributed to improved survival are high-dose melphalan with subsequent autologous stem cell transplantation, thalidomide, and a continuous improvement in supportive care.
