NEW YORK (Reuters Health), Dec 20 - A study by Swedish researchers demonstrates an association between nocturnal oxygen desaturation in obstructive sleep apnea (OSA) and the existence of a patent foramen ovale.
In a sleep apnea study, Dr. Magnus C. Johansson of Sahlgrenska University Hospital, Ostra, and colleagues found that oxygen desaturation was more common, and occurred in proportion to the frequency of respiratory disturbances, in patients with a patent foramen ovale.
OSA is associated with oxygen desaturation to a varying degree and affects up to 15% of middle-aged adults. Patent foramen ovale is present in 25% of the adult population, and may allow interatrial right-to-left shunting.
From a larger group of 209 diagnosed with OSA, the Swedish group identified 15 OSA patients with high proportional desaturation, and matched them with 15 OSA patients with low proportional desaturation. The 30 subjects were then screened for the presence of patent foramen ovale using transesophageal contrast echocardiography.
The researchers calculated the proportion of desaturation to respiratory events as the ratio of oxygen desaturation index to apnea-hypopnea index.
The presence of patent foramen ovale was more common in patients with more episodes of desaturation than in those with fewer episodes of desaturation, Dr. Johansson and colleagues report in the January issue of the European Respiratory Journal.
Specifically, nine out of 15 subjects (60%) with high proportional desaturation had a large patent foramen ovale compared with two out of 15 (13%) with low proportional desaturation.
"The present study," Dr. Johansson and colleagues write, "supports the hypothesis that interatrial shunting gives a substantial increase in the number of desaturations in OSA subjects with patent foramen ovale."
"Moreover, this may be the mechanism that explains the increased risk of stroke that is seen in OSA. If this link could be established, percutaneous closure of a patent foramen ovale may be a potential treatment option in the future," they conclude.
