Total and HDL cholesterol levels unrelated to heart failure

MedWire News: Total and high-density lipoprotein (HDL) cholesterol levels are not associated with the risk for heart failure, but are related to coronary heart disease (CHD) risk, show results from the Physicians' Health Study in the USA.

High levels of total cholesterol and low levels of HDL cholesterol are well recognized as risk factors for CHD, but their link to heart failure is less clear and previous studies have produced conflicting evidence.

Ravi Dhingra (Alice Peck Day Memorial Hospital, Lebanon, New Hampshire, USA) and colleagues investigated the potential association between heart failure and levels of total and HDL cholesterol in 10,813 men, with an average age of 68 years, recruited from the US Physicians' Health Study.

Over a follow-up period of approximately 6 years, 222 incident heart failure cases were recorded in the study population. Increases in total cholesterol of 36.7 mg/dl (0.95 mmol/l) and in HDL cholesterol of 15.3 mg/dl (0.40 mmol/l) were both associated with a 15% decrease in the risk for heart failure. However, these associations were abolished after accounting for standard risk factors for CHD such as body mass index.

There was also no statistically significant link between risk for heart failure and the ratio of total to HDL cholesterol. However, as expected high total cholesterol and low HDL cholesterol levels were associated with increased risk for CHD in the study population. Individuals in the highest quartile for total and HDL cholesterol had a 40% higher and 47% lower risk for CHD when compared with those in the lowest quartile, respectively. The results are reported in the American Heart Journal.

Some previous studies have linked an elevated total/HDL cholesterol ratio and raised triglycerides to onset of heart failure, but the authors point out that "most other studies did not find any association." They remark that, unlike similar previous studies, the current study was not limited by sample size.

Dhingra and colleagues conclude: "In our large healthy sample of males free of heart failure, total cholesterol and HDL cholesterol were not associated with incidence of heart failure." They add: "If confirmed, additional research studies are required to explore other techniques besides lowering cholesterol in primary prevention of heart failure."
