CT enterography can determine Crohn's disease severity

NEW YORK (Reuters Health), Nov 7 - Quantitative measures of bowel image enhancement at computed tomography (CT) enterography correlate with endoscopic and histological severity of ileal Crohn's disease, Minnesota-based researchers report in the November issue of Gut. Associations are also seen with C-reactive protein levels.

As lead investigator Dr. Joel G. Fletcher told Reuters Health, "because Crohn's disease is an inflammatory disease involving the small bowel, and because recent technical innovations permit faster and higher resolution CT scanning, modern CT scanners are now able to detect and measure inflammation involving and surrounding the small bowel wall."

To investigate such relationships, Dr. Fletcher and colleagues at the Mayo Clinic College of Medicine in Rochester, MN, studied CT enterography datasets from 143 patients undergoing ileoscopy.

Endoscopic severity score was significantly correlated with CT bowel enhancement, vascular enlargement of the vasa recta, and mesenteric fat density. Correlations with histological inflammation were strongest for bowel enhancement.

Compared with patients with increased bowel enhancement alone, serum C-reactive protein was significantly elevated in those with increased fat density -- an indicator of extraintestinal inflammation.

C-reactive protein did not correlate with inflammation limited to the small bowel wall, underscoring "the potential role of perienteric inflammation in C-reactive protein response to Crohn's disease." Further experiments, they add, "are needed to explore the mechanism linking C-reactive protein with mesenteric vascularity and fat."

In the future, concluded Dr. Fletcher, "we hope to quantitate Crohn's disease inflammation at imaging and combine this information with other markers to give gastroenterologists and surgeons a true idea of the inflammatory disease burden and type in every Crohn's patient."
