Research suggests that cynical and distrustful traits, along with stress and depression, significantly increase circulating inflammatory markers that are associated with cardiovascular disease (CVD) risk.
Stress and other psychosocial factors have previously been linked to an increased risk of CVD, but the reasons for this remain unclear, explain Nalini Ranjit, from the University of Michigan in Ann Arbor, USA, and colleagues.
To investigate further, the researchers used questionnaires to assess levels of cynical distrust, long-term stress, and depression in 6814 men and women aged between 45 and 84 years.
They also measured blood levels of the inflammatory markers interleukin (IL)-6, C-reactive protein (CRP), and fibrinogen.
The results, published in the Archives of Internal Medicine, indicated that levels of cynical distrust, chronic stress and depression were all associated with increased levels of inflammation.
The greatest association was seen for cynical distrust, which was linked to all three of the inflammatory markers studied, followed by chronic stress, which was associated with IL-6 and, to a lesser extent, CRP, and depression, which was associated only with IL-6.
Associations between these factors and inflammatory markers weakened after taking into account body mass index (BMI), and behaviors such as smoking, although they remained significant.
However, no associations remained after additionally controlling for socioeconomic position and diabetes.
The findings suggest that part of the link between these factors and CVD risk may be mediated by behaviors (for example, stress causing smoking which in turn elevates inflammation), and obesity and related metabolic problems.
In addition, the authors write, "socioeconomic position is likely to be an antecedent to psychosocial characteristics, as well as to other factors… associated with inflammation."
The team thus concludes that these cross-sectional results are compatible with a mediating role of behaviors, BMI, and diabetes, "although definite conclusions regarding mediation can only be drawn from longitudinal designs."
