"It has long been suspected that lipid metabolism is involved in the pathogenesis of acute pancreatitis," Dr. Daniel Closa from Institut d'Investigacions Biomediques, Barcelona, told Reuters Health. "Our work indicates a mechanism of action for several lipids in the exacerbation of the systemic inflammatory response associated with this disease."
Dr. Closa and colleagues examined the effect of the lipid fraction of pancreatitis-associated ascitic fluid generated in the early stages of acute pancreatitis. The findings are published in the May issue of Gut.
High levels of thiobarbiturate acid reacting substances in the pancreatitis-associated ascitic fluid lipid extract indicated an intense oxidative alteration in the lipids contained in ascitic fluid, the investigators say.]
These lipids obtained from pancreatitis-associated ascitic fluid reduced cell viability of macrophages and hepatocytes by 70%, the authors report, whereas lipids from white adipose tissue had no effect on cell viability.
Injection of complete pancreatitis-associated ascitic fluid or purified lipids obtained from pancreatitis-associated ascitic fluid into the portal vein increased serum AST, reflecting hepatotoxicity, but had no effect on serum lipase, indicating a lack of deleterious action in the pancreas, the researchers note.
The pancreatitis-associated ascitic fluid lipid extract was shown to abolish the macrophage inhibitory activity of endogenous PPAR-gamma agonists, thereby promoting an exacerbation of macrophage activation, the report indicates.
"This research helps to explain the resistance of acute pancreatitis to treatment," Dr. Closa said. "The release of a strong pro-inflammatory stimulus generated by the pancreatic gland in combination with interferences in the regulatory mechanisms due to the presence of several oxidize lipids make this pathology a formidable challenge in clinical practice."
"Our finding could open new therapeutic strategies for the treatment of the inflammatory response in acute pancreatitis," Dr. Closa said. "The generation of oxidized lipids interferes with the physiological regulation of the inflammatory response. Acting on these lipids in order to restore the endogenous control of inflammation could be a promising therapeutic approach."
