Introduction
Natural disasters (including earthquakes,floods, tropical cyclones, etc) are events in which significantnumbers of people are exposed to dangerous situations.1 The frequency of such disasters has increased alarmingly during the past decades.2-4 Evidence suggests that between a third and half of all the persons who are exposed to a disaster suffer from mental distress, including posttraumatic stress disorder (PTSD) and/or depression and/or anxiety disorders.5
Suicide also can be defined as “deliberate act of self harm leading to death.”6 It should be noted that one of the strongest risk factors for suicide tends to be mental disorders.6 In a meta-analysis, it has been shown that almost all of the mental disorders increase the suicide risk between fivefold and 15-fold.7 However, among all mental disorders, depression is more strongly associated with suicide.8
Given the link between exposure to natural disasters and becoming mentally distressed, on one hand, and between being mentally distressed and committing suicide, on the other hand, it would be possible that one finds a relation between exposure to natural disasters and committing suicide. This review, therefore, will not only seek such relation in the literature but also tries to highlight the most important methodological
issues of finding such connection.
Suicide after natural disasters, what literature tells us?
I started my literature review by searching using a well-known database, ie, PubMed (http://www.ncbi. nlm.nih.gov). It should be mentioned that PubMed offers access to MEDLINE, ie, the US National Library of Medicine’s (NLM’s) leading database that
covers almost all aspects of health and medicine. PubMed also provides access to other citations and to citations that go before the date that a journal was selected for MEDLINE.9
In my search strategy, I have chosen suicide, natural,and disasters as three key words and retrieved only 27 articles, from which there are only eight original articles that explicitly dealt with suicide after natural disasters. Most of these studies that were
conducted in a different variety of settings highlight that being exposed to a natural disasters might increase the rates of suicide,10-16 although one of them, the study by Krug et al.,15 later retracted some of its initial findings because of the problems in its statistical analyses.17 There is only one study that shows thereverse relation.18
It should also be emphasized that among the 27retrieved articles there are four original articles that dealt with suicidality, ie, suicidal ideation, plans, and deliberate self-harm (DSH) after natural disasters.DSH can be defined as nonfatal acts of self-poisoning and selfinjury, which refers to attempted suicide and parasuicide also.6 Similar to the previous one, three of four studies emphasize that being exposed to a natural disastersincreased suicidality,19-21 while only one study shows thereverse relation.22 Therefore, a review on the literatures shows that there are only a handful of articles that focus on the relation between suicide and natural disasters. Nevertheless, it would be possible that by changing the database or search strategy or its keywords plus hand searching of the selected journals or even by looking at the reference list of the retrieved papers, one can find an increased number of the relevant articles. However, there is still a scarcity of research in this area.
