Excess body weight is a recognized risk factor for VTE, the authors explain, but it has been unclear whether excess body weight increases the risk for recurrence.
Dr. Sabine Eichinger from the Medical University of Vienna, Austria and colleagues assessed the relationship between excess body weight and symptomatic recurrent VTE in 1107 patients who were prospectively observed after their first unprovoked VTE. Patients entered the study when their anticoagulation therapy was withdrawn.
Mean body-mass index (BMI) was significantly higher in patients who experienced recurrent VTE (28.5) than in those without a recurrence (26.9, p=0.01), the authors report.
They add that excess weight and recurrence were linearly related. The adjusted hazard ratio for each 1-point increase in BMI was 1.044.
The probability of recurrent VTE at 4 years was 9.3% among normal-weight patients, 16.7% among overweight patients, and 17.5% among obese patients, the researchers note.
If all the patients had been of normal weight, the investigators say, the risk of recurrence would be 26.8% lower than the actual rate observed in this group.
"Health care professionals should be encouraged to emphasize the need for weight control when counseling overweight or obese patients with thrombosis," the authors conclude.
They add, "Since the association between excess body weight and the risk of recurrence is linear, even a small weight loss might translate into a reduction of the risk of recurrent thrombosis. Patients of normal weight who have had a thrombosis should be told that weight gain might increase their future risk of venous thrombosis."
