T cell infiltration predicts breast cancer prognosis

Numbers of specific regulatory T cells (TR) in breast tumor biopsies offer a valuable indicator of disease prognosis and progression, researchers claim.

They found that high numbers of FOXP3-positive lymphocytes in breast biopsies predicted an increased risk of relapse in patients with ductal carcinoma in situ (DCIS), plus shorter relapse-free and overall survival in patients with invasive tumors.

"Although previous studies reported an increased pool of CD4+CD25+ TR in the peripheral blood of a small number of breast cancer patients, before our current study the clinical importance of tumor-infiltrating FOXP3-positive TR in diagnostic biopsies from this malignancy was unknown," the researchers say.

The results "are consistent with the hypothesis that the recruitment of tumor-infiltrating TR may enable malignant cells to evade the host immune response," they add.

Gaynor Bates (University of Oxford, UK) and colleagues measured the number of lymphocytes that stained positive for FOXP3 – the most specific TR marker – in microarray cores from 62 DCIS patients, 237 women with invasive breast cancer, and 10 with normal terminal duct lobular breast tissue.

TR numbers were significantly higher in in situ and invasive breast cancers than in normal breast tissue, and were higher still in the invasive tumors than DCIS, the researchers report in the Journal of Clinical Oncology.

Unlike conventional clinicopathologic variables, high numbers of FOXP3-positive TR identified patients with an increased risk of relapse at 5 years.

"These findings indicate that quantification of FOXP3-positive TR in breast tumors is valuable for assessing disease prognosis and progression, and that TR are an important therapeutic target for breast cancer," Bates et al conclude.
