NEW YORK (Reuters Health) Dec 13 - The results of a small crossover study suggest that supraphysiological testosterone levels may have detrimental effects on memory and brain function in elderly men, investigators at the University of Illinois at Chicago report.
Multiple lines of evidence link low testosterone levels to cognitive decline and dementia in aging men, Dr. Pauline M. Maki and her associates note in the November Journal of Clinical Endocrinology and Metabolism. Some studies have suggested beneficial effects of testosterone enanthate treatment and high serum testosterone, but the overall findings have been mixed.
The researchers conducted a double-blind, placebo-controlled crossover study with 15 healthy, eugonadal men ages 66 to 86, with no evidence of cognitive dysfunction. Intramuscular injections of testosterone enanthate 200 mg and sesame oil placebo were administered every other week for 90 days, with a 90-day washout between treatments. Neuropsychological testing and positron emission tomography were conducted 7 to 10 days after the final injection in each phase.
Total testosterone levels more than doubled after active treatment, the team reports.
But contrary to their expectations, the testosterone treatment was associated with declines of "medium effect size" in short-term verbal memory score (p < .05) and a nonsignificant decrease in composite verbal memory score (p = .09), as measured with the California Verbal Learning Test.
"Neuroimaging data provided insights into the neural basis for this decline in function," Dr. Maki and her associates state. "With testosterone treatment, relative activity decreased in the medial temporal lobe..., a pattern observed in preclinical Alzheimer's disease in a region that is critical for episodic memory."
Although this trial suggests that high doses of testosterone enanthate over long intervals may be harmful, Dr. Maki's group believes that "the results do not rule out potential benefits with other regimens, cognitive tests, or populations."
