"These potential benefits, however, should be weighed against expected adverse effects on risk of gout and other chronic diseases," lead author Dr. Xiang Gao and colleagues report in the April 1st issue of the American Journal of Epidemiology.
Multiple lines of evidence indicate that urate's potent antioxidant effects protect dopaminergic neurons, explain Dr. Gao, of the Harvard University in Boston, and colleagues. It remains unknown whether diet can influence serum urate and affect PD risk.
Their study included a sub-sample of 1387 men in the Health Professionals Follow-up Study who completed food-frequency questionnaires in 1986, at a mean age 61 years.
The investigators developed a dietary urate index based on intake of fructose and alcohol -- which are pro-uricemic - and dairy proteins and vitamin C -- which are anti-uricemic. A second index was tested that also included meat and fish/seafood intake because of their high content of purines, which are precursors of urate.
During mean follow-up of 14 years, there were 248 incident cases of PD. A higher dietary urate index was associated with a lower risk of PD. Comparing the top quintile with the bottom quintile of the dietary urate index, the relative risk of PD was 0.47, after adjusting for age, smoking, and other confounders (p = 0.0008).
The results were similar for the urate index that included meat and fish intake (RR 0.44 for highest urate index quintile vs the lowest, p for trend = 0.0002).
Dr. Gao's team concludes that their study supports "a possible neuroprotective effect of urate or its precursors in PD pathogenesis and suggests that dietary modulation of plasma urate may play a role in the prevention and treatment of PD."
