Arterial Function Altered Early in Migraine Patients

NEW YORK (Reuters Health) May 18 - Impaired arterial vasodilation and other abnormalities are seen in patients with migraine of recent onset, according to a report in the May 8th Neurology.

Migraine is associated with an increased risk of cardiovascular disorders, the authors explain, but the underlying mechanisms are unknown.

Dr. Jan N. de Hoon from K. U. Leuven, Belgium and colleagues investigated arterial structural and functional properties in 50 patients with a recent onset of migraine.

Brachial and aortic blood pressures were slightly but significantly higher among migraine patients than among controls, the authors report, but pulse pressures and heart rates did not differ between patients and controls.

Migraine patients also had a decreased diameter and compliance of their superficial muscular arteries, compared with controls, the results indicate.

After adjustment for age, height, mean arterial pressure, and heart rate, aortic augmentation index correlated independently with the presence of migraine, the researchers note, but aortic pulse wave velocity did not.

Flow-mediated dilatation response normalized to peak shear rate was significantly lower in migraine patients than in controls, the report indicates.

"Functional arterial properties are altered in patients with migraine of recent onset," the authors conclude. These results "strengthen our hypothesis that altered arterial properties are associated with migraine without being the consequence of vascular injury caused by repeated migraine attacks or vasoconstrictor drugs."

"Many questions remain," writes Dr. Gretchen E. Tietjen from The University of Toledo-Health Science Campus, Ohio in a related editorial, "but the study advances the science of migraine in an important direction, where the avenues of investigation may lead to improvement both in migraine and stroke prevention."

Neurology 2007;68:1563-1570,1555-1556.
