Aripiprazole Reverses Antipsychotic-Induced Hyperprolactinemia

NEW YORK (Reuters Health) Oct 01 - Treatment with aripiprazole, a dopamine partial agonist, reverses hyperprolactinemia induced by the antipsychotic haloperidol, according to a report in the September issue of the American Journal of Psychiatry.

"Adjunctive aripiprazole treatment can be a useful tool" in this setting, Dr. Joo-Cheol Shim from Inje University, Busan, Republic of Korea told Reuters Health.

Dr. Shim and colleagues investigated the effect of adjunctive aripiprazole versus placebo in an 8-week study of 54 patients with schizophrenia who had hyperprolactinemia related to haloperidol therapy.

Prolactin levels declined by 76.5% after 4 weeks of treatment with aripiprazole and by 84.2% after 8 weeks of treatment, the authors report.

Twenty-two of 26 patients treated with aripiprazole showed improvements in prolactin level into the normal range at week 8, compared with only 1 of 28 patients who received placebo.

Women began the study with higher prolactin levels than men, the report indicates, but the percent change in prolactin levels did not differ significantly between men and women.

Seven of 11 women with menstrual disturbances who were treated with aripiprazole regained menstruation, compared with none of the 14 women in the placebo group with menstrual disturbances.

Aripiprazole treatment had no significant effects on psychopathology, the investigators say, and no patients experienced any worsening of extrapyramidal symptoms or akathisia.

"We need to remember that aripiprazole adjunctive treatment is not first-line strategy for the treatment of hyperprolactinemia," Dr. Shim pointed out. "The traditional three strategies (reduction of antipsychotic dose, administration of adjunctive dopamine agonists, or discontinuation of current treatment with a switch to a different antipsychotic agent) should be considered first."

Dr. Shim added: "We are planning a project to compare the effect of prolactin-sparing antipsychotic drug compared to prolactin-elevating drug on the bone density in a prospective, long-term follow up study."
