NEW YORK (Reuters Health) May 22 - Obese adults with asthma are at risk for having more severe, persistent disease relative to their leaner counterparts, according to a large study reported Tuesday at the American Thoracic Society's 2007 International Conference in San Francisco.
In a telephone interview with Reuters Health, lead author Dr. Brian Taylor of Emory Crawford Long Hospital, Atlanta, said: "A lot of studies have clearly demonstrated that obesity is associated with asthma and we think actually that not only does obesity likely lead to asthma, but it may also lead to a more severe phenotype."
"Our study does not prove that," he emphasized, "but we were able to look at a number of covariables, such as age, race, income level, education, place of residence (urban vs rural area), and our findings stayed consistent independent of these covariables."
Dr. Taylor's team grouped 3,059 adults with asthma into three categories based on their BMI: normal weight, overweight, and obese.
Obese patients with a BMI of 30 or higher, compared with normal-weight adults, were 47% less likely to be in asthma remission and were 52% more likely to have severe persistent asthma, Dr. Taylor said.
They were also 66% more likely to report continuous symptoms, and 36% more apt to miss more than 2 days of work per year due to asthma than their non-overweight counterparts.
Obese adults with asthma were 36% more likely to use short-acting beta-agonists, 35% more likely to take inhaled corticosteroids, and 37% more likely to require controller medication.
Although the mechanisms of obesity and increased asthma severity are not entirely clear, evidence suggests that the hormone leptin, which is produced by fat cells and plays a role in weight regulation and asthma-related airway inflammation, may be involved.
Dr. Taylor and others are now involved in research looking at whether obese patients who undergo bariatric surgery experience a lessening of their asthma compared with obese patients who do not have the weight-loss surgery.
