NEW YORK (Reuters Health) May 30 - In a murine model of hind-limb ischemia, treatment with sodium nitrite can rapidly enhance ischemia-induced angiogenesis and restore blood flow, according to a report in the May 27th issue of the Proceedings of the National Academy of Sciences.

These findings suggest that sodium nitrite could be a useful treatment for peripheral artery disease and critical limb ischemia. The chemical already has a proven safety record and is also less expensive than other agents currently used treat these conditions.

"The most striking finding was how quickly sodium nitrite restored ischemic tissue blood flow and vascular growth," senior investigator Dr. Christopher G. Kevil told Reuters Health. "This is coupled with the fact that very low levels of sodium nitrite (low microgram/Kg doses) were needed to see the beneficial effects."

Dr. Kevil, a researcher with Louisiana State University Health Sciences Center in Shreveport, said that in prior studies, his team and others identified agents with pro-angiogenic effects, but that none of them worked as quickly as sodium nitrite. For instance, he said, sildenafil takes 7 days to augment angiogenesis, while sodium nitrite takes just 3.



Dr. Kevil explained that nitrite has typically been viewed as an end product of nitric oxide metabolism with no real significance. However, in tissues with low oxygen tension or pH, the nitrite can convert back to nitric oxide, thereby causing selective angiogenesis and arteriogenesis in ischemic tissues.

In the study, the beneficial effects of nitrite therapy were completely eliminated when the animals were treated with carboxy PTIO, a nitric oxide scavenger.

"Several questions remain to be addressed in future studies such as how exactly does nitrite stimulate endothelial cell proliferation and angiogenesis, and is sodium nitrite therapy effective at stimulating ischemia-induced angiogenesis in other organs (e.g. the heart and brain)," Dr. Kevil said.

He added that his team is now in the planning stages of clinical trials to investigate the effects of sodium nitrite in patients with peripheral vascular disease.
