NEW YORK (Reuters Health) Mar 09 - Women who are multiparous are at increased risk of melanoma, results of a case-control study suggest. However, the investigators report in the American Journal of Epidemiology for March, users of oral contraceptives and hormone replacement therapy appear to be no more likely than nonusers to develop melanoma.
To examine the roles of reproductive factors and exogenous hormone use in the etiology of melanoma, Dr. Margaret A. Tucker, from the National Cancer Institute in Rockville, Maryland, and her associates recruited women from San Francisco and Philadelphia who were diagnosed with incident, invasive cutaneous melanoma between January 1991 and December 1992. Randomly chosen control subjects were frequency-matched for age, race, and geographic study location.
The study included 318 women with melanoma and 395 controls.
In their logistic regression models to establish odds ratios (OR), the authors adjusted for demographics; other established risk factors for melanoma, including numbers of nevi and extent of freckling; and any use of exogenous hormones.
Compared with controls, women with melanoma were more likely to have ever been pregnant (OR 1.7, 95% confidence interval 1.0-2.6).
Among parous women, the OR was nearly tripled among women who had delivered two or more livebirths. Among women less than 55 years of age, having a livebirth within the 5 years before diagnosis also raised the risk.
There was also some evidence that changes in number or appearance of nevi during pregnancy was also associated with higher risk.
Neither total years of reproductive life nor use of exogenous hormones were associated with risk for melanoma.
"These findings should serve as a reminder that women (and their partners) should do regular skin self-examination to detect changing or otherwise suspicious pigmented lesions," Dr. Tucker and her associates conclude.
