Diuretics Do Not Prevent Preeclampsia

NEW YORK (Reuters Health) Feb 05 - Diuretics do not appear to prevent preeclampsia, contrary to the findings of an earlier nonsystematic review of randomized trials of diuretics in pregnancy, according to a report in the January 24th Cochrane Database of Systematic Research.

Dr. David Churchill from New Cross Hospital, West Midlands, UK and colleagues reviewed randomized controlled trials to ascertain whether the use of diuretics in pregnancy prevents the onset of preeclampsia.

There was no clear evidence of any difference in the rates of preeclampsia between women treated with diuretics and other women, the authors report, though there was a trend toward a reduction in preeclampsia among women receiving diuretics.

Diuretics also had no clear effect on the risk of caesarean section, perinatal death, or premature birth or gestation at delivery, the results indicate.

Women treated with diuretics were nearly 9 times as likely to have adverse drug events and nearly 6 times as likely to experience nausea and vomiting severe enough to stop treatment, the researchers note.

There were no significant differences in the diuretic and placebo groups in hypertension, severe preeclampsia, use of antihypertensive drugs, postmaturity, or low Apgar scores, the report indicates.

"There is insufficient evidence to draw reliable conclusions about the effects of diuretics for prevention of preeclampsia," the investigators write. "In addition, the incidence of side effects, particularly vomiting, was higher in the group receiving the diuretics."

"Therefore," the authors conclude, "at present, diuretics should not be recommended for this purpose in clinical practice."

The "first-line approach to reduce the risk of pre-eclampsia is low-dose aspirin," Dr. Churchill told Reuters Health. "While the evidence that diuretics prevent pre-eclampsia is lacking, they do not appear to cause any significant harm in terms of perinatal mortality."
