Repolarization measures predict mortality in post-MI conduction disturbance patients

Unconventional measures of repolarization duration significantly and independently predict mortality in post-myocardial infarction (MI) patients with conduction disturbances, US researchers report.

They say that using Rautaharju-formula QTRR, QRS and JTRR intervals are preferable to the traditional QTc interval for evaluating repolarization duration in patients with ventricular conduction defects.

Noting that identification of abnormal repolarization and measurement of the QT interval is influenced by the presence of a wide QRS complex and bundle branch blocks, the team used Bazett QTc and JTc (QTc minus QRS) and Rautaharju QT and JTRR formulas to measure repolarization duration in 3282 patients who had recently suffered an MI.

In 259 patients with conduction disturbances, 126 (3.8%) had indeterminate ventricular conduction disturbances, 126 (3.8%) had right bundle branch block (RBB, and 47 (1.4%) had left bundle branch block (LBB.

Those with LBBB had an excessively prolonged QTc interval, which was not observed with the JTRR interval, the team reports in the American Journal of Cardiology.

To determine the prognostic significance of each repolarization parameter for predicting mortality, the researchers divided the patients into high- and low-risk groups, with high-risk defined as a repolarization duration >75th percentile for QTc, JTc, JTRR, and QTRR, QRS intervals.

Adjusted hazard ratios with each were 1.65 (p=0.062) for the QTc interval, 1.46 (0.168) for the JTc interval, 1.71 (p=0.043) for the QTRR, QRS interval, and 1.70 (p=0.044) for the JTRR interval.

"Our study indicates a substantial overestimation of repolarization duration in patients with LBBB when using a traditional QTc formula," write Wojciech Zareba and colleagues from University of Rochester, New York.

They conclude: "Patients with LBBB, RBBB, or indeterminate ventricular conduction disturbances show longer repolarization duration than patients without these conduction disturbances, and QTRR,QRS and JTRR intervals reflect better than QTc repolarization duration in patients with conduction disturbances."
