NEW YORK (Reuters Health), Sep 27 - MRI of the hands can differentiate between patients with rheumatoid arthritis (RA) and patients with other diseases, even when x-rays are normal, symptoms are mild, and anticyclic citrullinated peptide (anti-CCP) antibodies are absent, researchers report in the September issue of the Journal of Rheumatology.
Early diagnosis of RA improves prognosis because treatment can be initiated before significant, irreversible damage occurs. But in the early stages, radiographs tend to be normal and anti-CCP antibodies may not yet be present.
In their prospective study, investigators at Lille University Hospital in France, led by Dr. Elisabeth Solau-Gervais, recruited 30 patients with polyarthritis with or without synovitis, with symptom onset no more that two years before. None had yet been treated with disease-modifying antirheumatic drugs, and patients had normal radiographs and no detectable levels of anti-CCP antibodies.
MR images of the hands and wrists were obtained to evaluate for synovitis and erosions.
According to the report, 16 patients were diagnosed with RA during the following year. The other patients were diagnosed with spondyloarthropathy, fibromyalgia, undifferentiated arthritis, sicca syndrome, hemochromatosis, polymyositis, or paraneoplastic syndrome.
The MRI images revealed significantly higher erosion scores for the metacarpophalangeal joint in RA patients than in the other group (19.3 versus 7.7), and for the second and third metacarpophalangeal joints (16.4 versus 5.7). MRI-detected erosions had a sensitivity of 64% and specificity of 70%.
Dr. Solau-Gervais and her associates conclude that when immunological markers and radiographs fail, "It is of interest to have another diagnosis tool such as MRI."
