Furosemide Inhalation Can Alleviate Dyspnea in COPD
NEW YORK (Reuters Health) Jul 18 - Inhalation of furosemide relieves exertional dyspnea in some patients with chronic obstructive pulmonary disease (COPD), according to a report by researcher in Canada.
Previous studies have shown that inhalation of nebulized furosemide alleviates the sensation of dyspnea provoked by different respiratory stimuli applied experimentally in healthy human subjects, the authors explain in the July issue of Thorax.
Dr. Denis E. O'Donnell and colleagues from Queen's University in Kingston, Ontario, compared the effects of inhaled furosemide or placebo on dyspnea intensity, airway function, ventilation, and other measures during high-intensity constant-load exercise in 20 patients with COPD.
Exercise endurance time increased by an average of 21% (1.65 minutes) after treatment with furosemide compared with placebo, the authors report, and dyspnea intensity during exercise decreased by 0.9 Borg units with furosemide compared with placebo.
At the symptom-limited end point of exercise, furosemide inhalation was associated with small but significant increases in peak oxygen uptake, CO2 production, minute ventilation, and mean expiratory tidal flow.
At baseline, those who responded to furosemide had significantly less severe expiratory airflow limitation and hyperinflation than did nonresponders, the investigators report.
"Nebulized furosemide was associated with consistent and potentially clinically important improvements in dyspnea intensity and exercise endurance in patients with advanced COPD," the researchers conclude. "These acute improvements in exertional dyspnea are multifactorial and could not be explained by alterations in ventilatory demand or pulmonary gas exchange."
"This study therefore supports the rationale for future assessment of inhaled furosemide as a therapeutic intervention for patients with COPD with incapacitating dyspnea," the authors add. Furosemide Inhalation Can Alleviate Dyspnea in COPD
