NEW YORK (Reuters Health) Oct 11 - Low levels of circulating endothelial progenitor cells (EPC) correlate with reduced erectile function in patients with coronary heart disease, clinicians from Germany report in the European Heart Journal for September.
"Levels of EPC are an independent predictor of erectile function in patients with coronary heart disease and may be the link between cardiovascular risk factors, endothelial dysfunction and ED," Dr. Magnus Baumhaekel from Universitatsklinikum des Saarlandes in Homburg/Saar, Germany told Reuters Health.
"Thus, increasing levels of EPC may help those patients with erectile dysfunction due to generalized atherosclerosis," the researcher said.
"ED is a frequent symptom of generalized atherosclerosis in the elderly, especially in patients with cardiovascular risk factors and diseases, the investigators point out.
Recent studies have shown that erectile dysfunction (ED) is related to endothelial dysfunction, "the first step of atherosclerosis," Dr. Baumhaekel said. "In the last few years, EPC were shown to be involved in repair mechanisms of the endothelial monolayer in atherosclerotic lesions."
Dr. Baumhaekel and colleagues looked for a possible association between EPC and ED in a study of 119 men with an average age of 64 and known coronary heart disease. The prevalence of ED was 59.7%.
In a multivariable analysis adjusting for age, diabetes, hypertension, BMI, smoking, left ventricular ejection fraction, use of statins, lower urinary tract symptoms and prior coronary intervention, men with low levels of EPC had worse erectile function than patients with high levels of EPC (p = 0.008).
A clinical implication would be to increase circulating levels of EPC in those patients, Dr. Baumhaekel said, noting that "recent studies demonstrated an increase of circulating levels of EPC in patients treated with statins or the PDE-V-inhibitor vardenafil. EPC were even increased in patients after recurrent physical exercise."
"Our findings," the researchers conclude, "identify EPC as a possible novel treatment target in patients with ED."
