Non-invasive MRI may predict heart attack risk

Researchers have developed a tool for measuring the molecular activity of blood vessels that may one day help identify patients at risk of heart attack and stroke.

The method uses magnetic resonance imaging (MRI) to gain a measure of the macrophage or white blood cell numbers in the arterial walls. The images not only indicate the macrophage activity but can also show whether this activity is unstable and therefore likely to trigger a heart attack or stroke.

The researchers used MRI to detect macrophage scavenger receptor (MSR)-targeted immunomicelles in the arteries of Apoliprotein (Apo)E-deficient atherogenic mice, and wild-type control mice.

Of note, they found that the MSR-immunomicelles produced a 79% increase in signal intensity of atherosclerotic aortas of the ApoE-null mice compared with just a 39% increase using untargeted micelles.

Confocal laser scanning microscopy confirmed the co-localization of fluorescent immunomicelles and macrophages in the atherosclerotic plaques, and the levels correlated with the MRI findings.

"Our study results clearly show that detecting and measuring macrophage levels using MRI could be an effective and non-invasive screening tool for what's becoming one of the leading public heath threats worldwide," said senior author of the study Zahi Fayad (Mount Sinai Medical Center, New York, USA)

He continued: "We have known that macrophages are red flags indicating inflammation in the blood vessels, and mounting evidence has cemented the causal relationship between inflammation and cardiovascular disease.

Yet we lacked the technology to measure this inflammation at the molecular level and predict who was at risk. Now, the technology is here, and our findings demonstrate that this new approach in cardiovascular screening not only works, but works very well."
