NEW YORK (Reuters Health) Jan 02 - Survival of patients with nonparalytic kyphoscoliosis is better with home mechanical ventilation than with long-term oxygen therapy alone, investigators in Sweden report.
Home mechanical ventilation and long-term oxygen therapy are commonly used to treat respiratory insufficiency in patients with kyphoscoliosis, the authors explain, but it is unclear which yields better outcomes.
Dr. Torbjorn Gustafson, from Skelleftea Hospital, and colleagues compared survival in 100 patients who were treated with home mechanical ventilation with survival in 144 patients treated with long-term oxygen therapy alone. The findings are published in the December 2006 issue of Chest.
At follow-up, 32% of patients receiving home mechanical ventilation were dead, compared with 76% of patients receiving long-term oxygen therapy, the authors report.
The survival advantage of home mechanical ventilation persisted after adjustment for age, gender, concomitant respiratory diseases, and blood gas levels, the results indicate.
Other factors associated with worse survival were being older than 70 years, the presence of a concomitant respiratory disease, low PaO2, and low PaCO2, the researchers note.
"The results of our study indicate that the vast majority of patients with kyphoscoliosis who are treated with oxygen therapy alone lose several years of potential survival due to ineffective therapy," the investigators write.
"We suggest home mechanical ventilation and not oxygen therapy alone as the primary therapy for patients with respiratory failure due to kyphoscoliosis, regardless of gender, age, and the presence of concomitant respiratory diseases," the authors conclude.
