LV wall motion abnormalities alone predict cardiovascular morbidity, mortality

MedWire News: Adults with echocardiographic left ventricular wall motion (WM) abnormalities but no overt cardiovascular disease have a substantially increased risk for cardiovascular morbidity and mortality, regardless of the presence of established risk factors, say scientists.

Left ventricular WM abnormalities have a recognized prognostic significance among patients with coronary or other heart diseases, but it is not clear whether such abnormalities predict adverse events in people without overt cardiovascular disease.

To investigate further, Richard Devereux, from Weill Medical College of Cornell University in New York, USA and colleagues studied 2864 participants from the second Strong Heart Study, who did not have clinically evident cardiovascular disease and had an average age of 60 years.

All of the participants underwent complete echocardiographic WM assessment, which indicated that 5% of them had focal hypokinesia, while 1.5% were found to have WM abnormalities, the team writes in the journal Circulation.

Over an average of 8.2 years of follow-up, 554 participants experienced fatal and nonfatal cardiovascular events, including myocardial infarction, stroke, coronary artery disease, and heart failure. A further 182 patients experienced cardiovascular death.

After taking into account age, gender, waist/hip ratio, systolic blood pressure, and the presence of diabetes mellitus, the team discovered that segmental WM abnormalities were linked to an increased risk for cardiovascular events, at an adjusted hazard ratio of 2.5, and cardiovascular death, at a hazard ratio of 2.6. For global WM abnormalities, the researchers calculated that the adjusted hazard ratio for cardiovascular events was 2.4, while that for cardiovascular death was 3.4.

“The present population-based study of adults without clinically evident cardiovascular disease demonstrates a strong relationship between the presence of WM abnormalities and cardiovascular events and death,” the team writes.

“These findings suggest that echocardiographic assessment of regional left ventricular dysfunction can identify adults without know cardiovascular disease who are at increased risk of future cardiovascular events.”
