Pulsed Dye Laser Effectively Treats Small Basal Cell Carcinomas

October 16, 2007 (Chicago) — Treatment of basal cell carcinoma (BCC) with the pulsed dye laser may be an effective alternative to Mohs micrographic surgery if the diameter of the lesion is less than 1.5 cm, according to a study presented at the American Society for Dermatologic Surgery (ASDS) 34th Annual Meeting.

Zeina Tannous, MD, chief of Mohs surgery at Veterans Affairs Hospital Boston, Massachusetts, and a dermatologist at Massachusetts General Hospital in Boston, presented her experience with the new treatment in 12 patients with 21 biopsy-proved BCCs (an update from her published abstract). Laser treatment consisted of 4 treatments (595 nm, 15 J/cm2, 3 ms) spaced at least 2 weeks apart. All malignant lesions were removed by disk excision for histologic analysis after laser treatment. As control subjects, 23 other BCCs underwent biopsy and excision only.

All laser-treated BCCs had at least a partial reduction, Dr. Tannous told Medscape Dermatology. Of the 21 laser-treated BCCs, 13 (62%) had a complete response, defined as no histologic evidence of disease. The response rate was significantly better than in control subjects (P = .0005), Dr. Tannous reported during the presentation. Tumors smaller than 1.5 cm had a complete response in 2 patients, regardless of histologic subtype (superficial or nodular). The average diameter of the tumor in complete responders was significantly smaller — 1.09 cm, compared with 2.81 cm for tumors that had only a partial response (P < .005). Adverse effects of laser treatment included mild skin crusting in a few patients, which healed completely.

The mechanism of action, Dr. Tannous speculated, is that the laser cuts off the vascular nutrient supply to the cancer. BCCs have abnormal vasculature that could be a treatment target for the pulsed dye laser, according to her abstract.

"I think pulsed dye laser is very promising for small basal cell carcinomas," she told Medscape Dermatology, "but it's premature to consider recommending this treatment instead of excision."

Sherrif Ibrahim, MD, PhD, from the University of Rochester, New York, said in an interview, "I think there's a big future for laser treatment of cutaneous malignancy. It has the huge benefit of being less invasive and less time-consuming than Mohs surgery."

However, Dr. Ibrahim, who was not part of the study, cautioned that further refinement of laser treatment is needed. "It has a long way to go to reach the 99% cure rate of Mohs [techniques]," Dr. Ibrahim said. "It takes a lot to replace the current standard of care."

Dr. Tannous said she and her coinvestigators plan to study more patients with different laser parameters and longer follow-up to monitor for recurrence. It is possible that BCCs larger than 1.5 cm need more than 4 laser treatment sessions, she noted.
