NEW YORK (Reuters Health) Jun 05 - Both von Willebrand factor (vWF) and tissue plasminogen activator (t-PA) are considered markers for endothelial dysfunction and may be part of the "common soil" of coronary heart disease and type 2 diabetes. However, vWF does not appear to be a risk factor for new-onset type 2 diabetes, while t-PA is a strong and independent risk factor.

Dr. S. Goya Wannamethee, of the Royal Free and University College Medical School in London, and colleagues conducted a prospective study of 3562 non-diabetic men ages 60 to 79 years. During an average of 7 years of follow-up, 162 cases of type 2 diabetes developed, according to their report in the May issue of Diabetes Care.

Men with t-PA levels in the upper tertile had a nearly three-fold higher risk (relative risk 2.98) of type 2 diabetes compared men in the lowest tertile, after adjusting for lifestyle factors and waist circumference.

A weaker but still statistically significant association was found for vWF and new-onset type 2 diabetes (relative risk 1.24).

Both markers of endothelial dysfunction were linked with inflammation, specifically interleukin-6 and C-reactive protein, and with abnormal hepatic function (elevated gamma-glutamyl transferase) and insulin resistance.

Dr. Wannamethee and associates found that t-PA had a stronger association with adiposity, hepatic function, and insulin resistance than did vWF, and that it was also significantly and inversely associated with adiponectin.

After adjusting for those risk factors, the relative risk of incident diabetes was 2.19 for elevated t-PA but only 1.06 for vWF.

"Subsequent adjustment for insulin attenuated the association further, but t-PA was still associated with a significant increase in risk (of incident type 2 diabetes) of 1.66," the researchers conclude.
