MedWire News: Researchers have demonstrated a V-shaped association between low-density lipoprotein (LDL) cholesterol and cancer incidence in patients with Type 2 diabetes mellitus.

Juliana Chan and colleagues from the Chinese University of Hong Kong say that several studies have "demonstrated an inverse relation between serum total cholesterol and cancer incidence and mortality in the general population."

However, "few studies have investigated this relation among patients with Type 2 diabetes mellitus," they add.

Chan and team recruited a group of 6107 patients with Type 2 diabetes who were cancer-free at baseline. The researchers measured serum lipids and anthropometric measurements at enrolment.

The participants were then followed up for an average of 4.9 years, during which 270 individuals developed cancer, giving an incidence rate of 4.4%.

The authors then assessed associations between LDL cholesterol concentration and cancer incidence.

As published in the Canadian Medical Association Journal, Chan et al found a V-shaped association between LDL cholesterol level and cancer incidence in participants not receiving statin therapy.

Patients with a LDL cholesterol concentration of 2.80-3.79 mmol/l (108.36-147.06 mg/dl) had the lowest risk for cancer, while those who had LDL cholesterol below 2.80 mmol/l (108.36 mg/dl), and those with levels of 3.80 mmol/l (147.06 mg/dl) or above were at higher risk.

Compared with the intermediate LDL group, hazard ratios for cancer in the low and high LDL groups were 1.74 and 1.87, respectively.

The researchers note that the associations persisted after adjustment for potential confounders and exclusion of individuals with less than 2.5 years of follow-up.

In an accompanying editorial, Eric Ding and Frank Hu from Harvard University in Boston, Massachusetts, USA cautioned: "A V-shaped association between LDL cholesterol and cancer risk suggests that multiple mechanisms are involved. LDL cholesterol is unlikely the sole or direct causal factor."

They added that "further investigations are required about the implications of cholesterol-lowering drug therapy on cancer risk and the ability of LDL cholesterol to predict the development of specific cancers in the general population as well as in patients with diabetes."
