Brain Stimulation Limits Side Effects of Parkinson's Drugs

NEW YORK (Reuters Health) Feb 09 - Subthalamic nucleus (STN) deep brain stimulation modulates afferent inhibition in Parkinson's disease and also appears to offset untoward side effects of medication, Canadian and Swedish researchers report in the January 30th issue of Neurology.

"Although STN stimulation is increasingly being used to treat Parkinson's disease, how it works is not fully understood," senior investigator Dr. Robert Chen told Reuters Health. "Our findings suggest STN stimulation may have a direct effect of reducing the side effects of Parkinson medications such as dyskinesia, and not just through reduction in the dosages of medications."

Dr. Chen of the University of Toronto and colleagues came to this conclusion after they studied the effects of STN stimulation in 7 Parkinson's patients and age-matched controls.

The patients were studied in an off-medication session followed by an on-medication session. The stimulator was switched on or off in random order in each session.

The researchers note that previous studies suggested that reduced short latency afferent inhibition in Parkinson's disease reflects the adverse effect of dopaminergic medications and reduced latency afferent inhibition may be related to nondopaminergic manifestations of the disease.

In the on-medication session, the team found that short latency afferent inhibition was reduced in the stimulator off condition and was restored by STN stimulation. Latency afferent inhibition was partially normalized by STN stimulation in the medication-on condition.

"It is known that symptoms that respond to dopaminergic drugs... are improved by STN stimulation," concluded Dr. Chen. "Our study suggests that symptoms that do not respond to dopaminergic medications may also be improved by STN stimulation."
