NEW YORK (Reuters Health) Apr 29 - Blood pressures obtained with an automated sphygmomanometer are as accurate as measurement with a standard manual mercury device, Canadian researchers report.

Dr. Martin G. Myers and colleagues at the University of Toronto in Ontario recorded blood pressures in 250 individuals, using both an automated sphygmomanometer (BpTRU Medical Devices) and a manual mercury sphygmomanometer, in random order.

Subjects rested for five minutes prior to the first measurement. Four consecutive readings 1 minute apart were taken by mercury sphygmomanometer; the first measurement was discarded and the mean of the following three readings was used. With the automated device, six reading were taken at 1 minute intervals, with the first reading discarded and the mean of the other five readings used.

The mean blood pressure with the automated device was 115/71 mm Hg, compared with 118/74 mm Hg with the manual device, the researches report in the March issue of the American Journal of Hypertension.

A systolic blood pressure of 140 mm Hg or higher was recorded in 16 automated readings and in 19 manual readings. A diastolic blood pressure of 90 mm Hg or higher was recorded in nine automated and 14 manual readings.

Statistical analysis showed that automated and manual readings were not significantly different, Dr. Myers and colleagues say.

"Conventional manual BP readings can be replaced by readings taken using a validated, automated BP recorder in population surveys," the investigators conclude. "The slightly lower readings obtained with the BpTRU device may be a more accurate estimate of blood pressure status."

"This information is important and definitely points to the future of office BP management," Dr. George S. Stergiou of Sotiria Hospital and Third University in Athens, Greece, comments in an accompanying editorial.

However, Dr. Stergiou cautions that the lower readings seen with the automated device need further study. It may be related to the device itself or it may indicate "the elimination of the white coat effect," he says.

"Any change in the conventional methodology should be carefully tested and supported by evidence showing clear improvement in the diagnostic ability of the method," Dr. Stergiou writes.
