NEW YORK (Reuters Health) May 15 - Plasma levels of two amyloid beta proteins -- Abeta1-40 and Abeta1-42, both of which are important components of senile plaque -- do not appear to be useful biomarkers for development of Alzheimer's disease (AD), according to research published in the May 6th issue of Neurology.

Dr. Oscar L. Lopez from the University of Pittsburgh and colleagues note in their report that amyloid beta proteins can be found in the plasma and are increased in both familial and sporadic forms of AD. The purpose of their study was to see if amyloid beta proteins are increased before the onset of dementia, and stay elevated thereafter.

The investigators examined the association between plasma Abeta1-40 and Abeta1-42 levels and incident Alzheimer's disease in 232 adults who had normal cognitive function in 1998-1990 and 42 who had mild cognitive impairment during this time frame. They determined Abeta1-40 and Abeta1-42 levels in 1998-1999 and again in 2002-2003. The mean age of the study subjects at baseline was 79.3 years.

In unadjusted models, elevated plasma Abeta1-40 and Abeta1-42 were associated with conversion from normal cognition to dementia, Dr. Lopez and colleagues report.

However, after adjusting for relevant covariates, including age, kidney function, cerebrovascular disease, APOE-4 status, and cognitive state, there was no significant association between elevated plasma levels of these amyloid beta peptides and incident Alzheimer's disease.

"Therefore, amyloid beta plasma levels seem to be weak predictors of conversion to Alzheimer's disease in cognitively normal individuals," the researchers conclude.

However, the finding that plasma Abeta1-40 levels were elevated in subjects with mild cognitive impairment who transitioned to dementia "suggests that at least some of the amyloid was due to the Alzheimer's disease process," Dr. Lopez and colleagues note.

"Thus, there may be a brief window, during the mild cognitive impairment phase, when measurement of plasma amyloid may be useful," they conclude.
