NEW YORK (Reuters Health) Feb 14 - Aortic dilatation during childhood in patients with Marfan syndrome can be reduced by beta-blocker therapy, according to a report in the February 1st American Journal of Cardiology.
"You should treat the aortic fragility as soon as possible," Dr. Guillaume Jondeau told Reuters Health, "even if aortic dilatation is not present."
Dr. Jondeau from Hopital Ambroise Pare Assistance-Publique Hopitaux de Paris, France and associates investigated the impact of beta-blocker therapy on the evolution of aortic diameter in 155 children under 12 years with Marfan syndrome. About half were not treated with beta-blockers.
Although baseline aortic diameters were significantly larger in children treated with beta-blockers than in untreated children, aortic diameters were not significantly different by the end of the study.
Aortic root dilatation progressed significantly more slowly in children who received beta-blocker therapy, the authors report, so that by age 18, aortic root diameters were identical to those in the control group.
Valvular heart disease remained largely unchanged during the study, the researchers note, and there were very few events during follow-up.
Three children died and five underwent aortic surgery in the control group, the report indicates, compared with one death and two aortic surgeries in the treatment group.
"This retrospective study strongly suggests that the beneficial effect of beta-blocker therapy, already demonstrated in adults with Marfan syndrome, is also observed in children," the investigators conclude.
"Screening families is crucial to find affected patients who would not know it," Dr. Jondeau emphasized.
